CMS adds rural emergency hospitals to its survey database worksheet
Oct 11, 2026 edition · GC-2026-0297 · Published 2026-10-11 · Version 3 · Final
CMS updates its internal hospital survey worksheet to incorporate rural emergency hospitals and streamline information collection. The October 9 revision preserves surveyor responsibility and facility verification, with required fields controlling survey-kit upload rather than creating a new facility self-filing deadline.
What happened
CMS Transmittal 249 revises State Operations Manual Exhibit 286, the Hospital, Critical Access Hospital, and Rural Emergency Hospital Database Worksheet, effective and implemented October 9. CMS identifies streamlined information collection and inclusion of REHs as the changes. The worksheet is completed by the State Agency surveyor as an internal tool and should not be given to a provider to complete independently. Surveyors may work jointly with facility staff but must verify the information with the hospital, CAH, or REH.
The worksheet records facility type, emergency services, accreditation, licensed beds and procedure rooms, training affiliations, co-location, off-site locations, and affiliated-provider identifiers. Asterisked fields are mandatory, and the survey kit cannot upload to the national database until required entries are complete. This upload condition concerns the survey process, rather than a new universal provider submission date. The transmittal also limits its revision date to new red italicized material; the presence of an older instruction in the reproduced exhibit does not establish that CMS newly adopted that instruction on October 9.

Why it matters
Surveyors verify facility, service, off-site and affiliated-provider information for certification records. The worksheet is a State Agency survey tool; facilities do not receive a new nationwide self-filing deadline. REHs cannot furnish ordinary acute inpatient services, apart from an elected distinct-part SNF.
Recommended action
Keep accurate CCNs, accreditation status, service inventories and off-site affiliations available for surveyors, and reconcile errors with the State Agency.
