FDA seeks input on ibogaine trial safety and design
Oct 11, 2026 edition · GC-2026-0296 · Published 2026-10-11 · Version 3 · Proposed
FDA's October 6 ibogaine RFI requests November 20 comments on early-trial design and safeguards. It describes preliminary approaches to serious cardiac and other risks, while reserving individualized IND review and expressly stating that the RFI is neither final guidance nor legally enforceable trial authorization.
What happened
FDA opens a docket on design and safety considerations for early-phase ibogaine trials, including dose selection and escalation, eligibility, care setting, monitoring, stopping rules, and oversight. The notice describes planned federally supported research in adults with opioid-use disorder or PTSD and the agency's interest in data informing IND review. FDA reports substantial cardiovascular concerns, including QTc prolongation and potentially fatal arrhythmia, alongside other adverse effects and major limits in available product and participant-level evidence. Comments, data, and information are requested by November 20.
The agency's preliminary approach considers small sequential groups receiving a single ascending dose under intensive inpatient monitoring, with qualified cardiac-response capability and appropriate mental-health oversight. It discusses drug characterization, metabolism, safety review, and follow-up as design issues rather than presenting a generally approved protocol. FDA expressly says it has made no final decision on which trial characteristics would permit an IND to proceed and that the RFI establishes no enforceable requirements or final regulatory expectations. Federal funding or publication of this consultation therefore does not authorize routine clinical treatment or predetermine an individual IND decision.

Recommended action
Clinical research and regulatory teams should assess applicable IND protocols and consider evidence-based comments on monitoring, eligibility and stopping rules.
