Federal EMS agencies seek comments on prehospital blood transfusion barriers

NHTSA seeks November 5 comments for a federal EMS consultation on expanding prehospital blood transfusion. Its questions address operational barriers, technology, data linkage, and training; the national 2028 objectives are consultation goals rather than enforceable provider quotas.

What happened

The October 6 notice requests information on behalf of the Federal Interagency Committee on Emergency Medical Services about implementing prehospital blood-transfusion programs nationwide. NHTSA invites public, private, governmental, academic, and professional perspectives on barriers and potential solutions across state, local, tribal, and territorial EMS systems. Comments are due November 5. The notice expressly states that the RFI is for information gathering and planning and is not a request for proposals or invitation for bids.

The questions connect transfusion capability with emergency dispatch, trauma care, blood banking, and reliable logistics. They seek views on automated crash notification, structured pre-arrival instructions, linkage of NEMSIS and trauma-registry data, military/civilian cooperation, cold-chain management, and EMS education. The stated 2028 national goals provide a planning framework for that feedback, including broader transfusion access and outcome measurement. They do not themselves award funding, establish a new clinical authorization, or order every EMS agency to implement a transfusion program by that year.
