HRSA seeks comments on transplant forms and organizational identifiers
Oct 11, 2026 edition · GC-2026-0293 · Published 2026-10-11 · Version 3 · Proposed
Two October 9 HRSA notices seek OMB review and November 9 comments on transplant forms and organizational identifiers. The NPI/CCN proposal covers organizations already holding those identifiers and does not require applicants to obtain them or supply individual practitioner NPIs.
What happened
HRSA's OPTN information-collection revision proposes new Patient Safety Contact Management and Patient Transfer forms for organization contacts and post-transplant follow-up transfers. It would remove the superseded VCA Transplant Candidate Registration, Death Notification Registration, and Deceased Donor Death Referral forms, with the described replacement systems carrying those data. Minor form edits are also proposed. The October 9 notice invites further comments through November 9 during OMB review; proposed additions and removals should not be described as final approval of every new form.
The second notice proposes collecting organizational Type 2 NPIs and CMS Certification Numbers during Grants.gov applications from organizations that already have them. Individual Type 1 NPIs are excluded, and organizations need not acquire an identifier to apply for or receive funding. HRSA describes research, evaluation, data-linkage, accountability, and oversight uses and recognizes that an NPI, facility CCN, and legal awardee may identify different levels of an organization. The proposal is not an individual eligibility-verification instrument or proof that enumeration validates clinical licensure. Its November 9 comment date is separate from any later implementation instruction.

Why it matters
The identifier proposal covers existing organizational identifiers only; it does not require obtaining an NPI or CCN, collecting individual NPIs, or using the collection to determine assistance eligibility.
Recommended action
Transplant data and grant owners should compare proposed instruments with existing workflows and decide whether to comment.
