ORR proposes health forms and limited continuity-of-care sharing
Oct 11, 2026 edition · GC-2026-0295 · Published 2026-10-11 · Version 3 · Proposed
ORR proposes revised mental-health and onsite health forms and specific continuity-of-care sharing for children identified for repatriation. The October 5 PRA notice seeks November 4 comments and ties the described DHS sharing to relevant care needs, rather than every health record or every child.
What happened
ORR requests a three-year information-collection extension with changes to its Mental Health Assessment and Onsite Health Intervention forms. The proposal would merge active-TB and non-TB public-health forms into one onsite instrument addressing exposures, triage, and standing medication orders. Mental-health revisions include reducing duplicate follow-up history, strengthening accommodation fields, recording ordered or completed tests, clearer visit instructions, and a provider acknowledgment concerning ORR sharing. The forms support health and care documentation for unaccompanied children; completed information is entered in ORR's secure system. Comments are due November 4.
The notice also describes sharing relevant health information with DHS when a child in ORR custody has care needs and is identified by DHS for repatriation. The specified information includes identifiers, active management needs, allergies, current medications and doses, isolation or quarantine requirements, accommodations and equipment, and health-related travel restrictions. The stated purpose is continuity of recommended care and accommodation. This limited proposed use should not be restated as authority for unrestricted record transfer, a general private-provider disclosure mandate, or an already completed final rule replacing all applicable confidentiality requirements.
