What happened

AHRQ published an October 2, 2026 notice that Hospital Rep Safety Solutions Corp., PSO P0264, had been delisted for cause after failing to correct a deficiency. The delisting took effect at midnight Eastern Time on September 9. Publication and effectiveness therefore occurred on different dates, and the notice reports an action already effective by October 4. It identifies a failure to correct a listing deficiency without supplying clinical incident details or establishing a Medicare provider exclusion.

The notice explains that the Patient Safety Act and implementing rule provide a framework for reporting patient safety work product to listed PSOs for aggregation and analysis under applicable privilege and confidentiality protections. Delisting can also occur through voluntary relinquishment or expiration, but AHRQ identifies this event specifically as for cause. Organizations should therefore verify the exact PSO identity and effective date in their arrangements and obtain legal advice on reporting and protections. The notice alone does not establish that every historical communication loses protection or authorize disclosure of patient safety information.

Why it matters

A PSO status change can affect patient-safety arrangements and legal analysis, but delisting should not be equated with a universal loss of historical confidentiality.
