What happened

DMHC executed a September 27, 2026 agreement with the commission operating Central California Alliance for Health in matter 26-145, addressing the quarterly grievance report for the quarter ending December 31, 2025. The report was due January 30, 2026 and was filed March 26, 55 days late. DMHC emailed the plan on March 5 about the overdue report; the plan did not respond until the filing date. The agreement records acknowledged noncompliance and a $2,750 administrative penalty, with limitations on admission use in civil or criminal proceedings.

The letter describes quarterly reporting on grievances pending and unresolved for 30 days or more, with filing no later than 30 days after each quarter. Its corrective plan required the plan to address the root cause of both the filing delay and its delayed response to DMHC, and to document steps for timely future filings. Both deliverables are expressly recorded as completed on August 19, before the agreement was executed. The record therefore distinguishes the original missed reporting deadline, completed corrective work and the later enforcement agreement; it does not establish that the penalty had been paid.
