OIG questions $454.9 million Wisconsin school-based Medicaid reimbursement
Oct 4, 2026 edition · GC-2026-0194 · Published 2026-10-04 · Version 2 · Final
OIG questioned an estimated $454.9 million in Wisconsin school-based Medicaid reimbursement, separating a $67 million refund recommendation from $387.9 million to refund or substantiate.

What happened
OIG's Wisconsin school-based Medicaid audit was issued September 30 and posted October 2, 2026. Reviewing $505.5 million in services over four years, OIG estimated $454.9 million may have been improperly claimed. It separated at least $67 million associated with unsupported random-moment time-study coding and unlicensed providers from an additional $387.9 million tied to unsupported allocation ratios. The latter was not framed as an unconditional finding that the entire amount must be repaid regardless of further support.
OIG recommended refunding the $67 million estimate and either refunding the $387.9 million or supplying documentation reasonably supporting the ratios. It also noted possible additional unallowable costs because sampling omitted moments at the beginning of the school year and made procedural recommendations. Wisconsin generally disagreed with the findings, disputed some recommendations and described improvements, with different responses to the individual findings. This is an audit with disputed conclusions and documentation questions, not a settlement or court judgment. Review should preserve the distinction between time-study coding, provider qualifications, allocation support and the sampling frame.
