OIG hospital audits identify Medicare overpayments and recurring claim-control failures

Separate OIG hospital audits identified $256,926 in sampled Methodist overpayments, with an estimated $12.4 million for selected services, and $38,676 at McLeod. The reports distinguish disputed recommendations and agreed refunds from a judgment or confirmed recovery.

What happened

OIG’s September 17 Methodist Hospital audit examined 100 inpatient and outpatient claims totaling about $1.43 million for services in 2020 and 2021. Twenty-seven did not fully comply with Medicare billing requirements, producing $256,926 in net overpayments in the reviewed claims. OIG separately estimated at least $12.4 million in net overpayments for selected service types within a $62 million payment population. That larger figure is an estimate, not the amount found by adding the sampled errors. OIG recommended a refund, consideration of later-period internal audits and additional training. Methodist disagreed with the refund and internal-audit recommendations and said it was willing to provide education.

The September 16 McLeod Regional Medical Center audit reviewed 115 claims totaling about $1.52 million for the same 2020–2021 period. Seventeen did not fully comply, resulting in $38,676 in net overpayments. OIG again identified failures to follow billing policies within the selected risk areas and recommended a refund, consideration of later-period audits and training. McLeod agreed with most findings and said it would refund the agreed overpayments, without formally concurring or nonconcurring with the recommendations. Both hospitals were selected for potentially high-risk claims. Their results therefore support targeted control testing rather than treating the sample error rates as representative of every hospital claim, or the recommendations as court judgments or completed recoveries.

Why it matters

Keep estimated extrapolation separate from sampled overpayments. The reports identify failures to apply hospital billing policies; they do not order every hospital to repay the same amount.

resolution
OIG recommends a $12.4 million estimated refund from Methodist, which disputed the refund and internal-audit recommendations; McLeod was advised to refund $38,676 and said it would return agreed overpayments. No court judgment or collected recovery is established.

Operational lessons

  • Use targeted claim audits to identify systemic payment risk.
  • Escalate credible overpayment findings promptly.
  • Document coding, billing, and repayment corrective actions.
