What happened

DOJ’s September 30 announcement says Signal Diagnostics agreed to pay $20.5 million to resolve False Claims Act allegations concerning HRSA’s COVID-19 Uninsured Program. The program reimbursed eligible providers for services to uninsured people from approximately May 2020 through April 2022. Signal submitted testing and specimen-collection claims from October 2020 through March 2022. The government’s retention allegation covers a different period, January 2022 through May 2023, when Signal allegedly avoided obligations to repay amounts received for patients who had insurance.

According to DOJ, an internal audit begun around January 2022 identified ineligible claims. The alleged errors included duplicate profiles for the same patients with conflicting insurance information and claims submitted despite alternative coverage being on file. The government contends that Signal continued auditing and calculating the overpayment while knowingly and improperly avoiding repayment through May 2023. That sequence makes the connection between eligibility review, quantification and refund decisions central to the case, rather than treating the audit itself as the completed remedy. The settlement resolves allegations only, with no determination of liability; it does not reopen the closed reimbursement program or establish that settlement funds were already collected.

Why it matters

An internal eligibility audit needs a controlled path from identified ineligible claims to quantified amounts and repayment decisions. This closed-program case does not reopen HRSA reimbursement.
