CMS refreshes hospice telecommunications recertification reporting guidance

CMS’s September revision refreshes hospice telecommunications claim instructions without substantive article changes. January 1 service applicability and January 4 systems implementation remain separate dates.

What happened

CMS revised MM14495 on September 2 to update the change-request release date, transmittal numbers and links, expressly stating that the article’s substance did not change. The instruction applies from January 1, 2027, with January 4 systems implementation. It concerns hospice claims reporting a face-to-face recertification encounter conducted through telecommunications by a hospice physician or hospice nurse practitioner. The revised article is implementation guidance, rather than a newly announced September clinical eligibility standard.

The instructions use G0679 and line-item detail, with a separately dated line for each service under the appropriate discipline’s revenue code. MACs return claims using G0679 with bill types other than 81X/82X or a revenue code other than 0657. The descriptor treats the encounter as an administrative expense; reporting the code does not create a separately payable clinical visit. CMS also explains the statutory basis for identifying virtual-platform encounters involving specified contractual or payment arrangements. Template changes need to preserve those conditions and the distinction between service applicability and system implementation.

Why it matters

A reporting code, an encounter’s clinical eligibility and a payment entitlement are separate questions. Claim templates should reflect the actual combinations and dates.
