AB-1682 Health care coverage: scalp cooling

Pending AB 1682 would cover prescribed FDA-cleared scalp cooling for chemotherapy-related hair loss in large-group coverage and conditionally in Medi-Cal.

What happened

AB 1682 proposes coverage of prescribed scalp cooling to reduce the incidence or severity of alopecia before, during or after chemotherapy involving hair-loss-inducing agents. The large-group contract and insurance provisions would apply to contracts issued, amended or renewed from January 1, 2027. They permit cost sharing, but on terms no less favorable than oncology supportive care under the same coverage. The proposal excludes specialized coverage and does not apply those commercial provisions to Medi-Cal managed care.

A separate Medi-Cal provision would cover the service subject to medical necessity, utilization controls, necessary federal approvals and available federal financial participation that is not jeopardized. Across the proposed provisions, scalp cooling means an FDA-cleared device or system; non-cleared cold caps and similarly marketed products are excluded. DHCS could issue implementation instructions without further regulatory action. Pending status at the coverage cutoff means these details support benefit-design preparation, rather than a representation that every chemotherapy patient already has this new entitlement.

Why it matters

Large-group contract timing, device clearance and Medi-Cal’s federal conditions prevent the proposal from being read as one blanket benefit across every product.
