CMS announces broad state participation in the GENEROUS Medicaid drug payment model and a September 30 signing date

CMS reports applications from every state, DC and Puerto Rico, but fewer signed GENEROUS agreements. The September 30 signing date remains ahead; applications alone do not establish each state’s participation terms.

What happened

CMS announced September 18 that all 50 states, the District of Columbia and Puerto Rico had applied for the GENEROUS Medicaid drug-payment model. Forty states and Puerto Rico had signed participation agreements at the time of that release, with the remainder given until September 30. CMS describes the model as having launched in January 2026 for five years and estimates $64.3 billion in taxpayer savings over ten years. The estimate is a forecast, and the broad application count is distinct from the smaller signed-agreement count reported on September 18.

Participating manufacturers make covered outpatient drugs available to participating state programs at most-favored-nation prices through supplemental rebates invoiced under state agreements. CMS monitors pricing accuracy and shares in the rebates through a reduction in the federal share of Medicaid payments. Those mechanics create state, manufacturer and federal accounting dependencies rather than an automatic new provider payment rate. At this September coverage cutoff, the September 30 signing date remains ahead. The release does not establish California’s final agreement status or that every applicant will sign. State-specific terms and operational instructions remain necessary to assess downstream effects.
