AB 2160 directs updated Medi-Cal lactation guidance by July 1, 2027, including access without prior authorization, prescription or referral.
What happened
California approved and filed AB 2160 on September 27. DHCS must issue updated lactation guidance by July 1, 2027 clarifying a continuum from lactation education and basic support through clinical consultation. The guidance must identify standard billing and reimbursement procedures and codes that reflect the covered services, and clarify managed care plans’ coverage and reimbursement responsibilities. It must also specify that lactation services are not subject to prior authorization and require neither a prescription nor referral. These are express instructions for the guidance rather than a basis to assume every claims system has already changed as of October 4.
The statute also directs plans to report how many International Board Certified Lactation Consultants are accessible in-network to members who need clinical consultation. DHCS must seek stakeholder input before issuing the guidance and may implement through letters, bulletins or similar instructions pending regulations. Implementation is conditioned on federal financial participation and necessary federal approvals. Providers and plans therefore need to follow the actual approved benefit and billing instructions while preparing for the required access, coding and network-information changes, without treating the guidance date as proof those conditions have already been satisfied.

Why it matters
Lactation access can be impaired by referral or authorization edits and unclear coding; the required guidance also makes clinical consultant availability measurable.
Recommended action
Inventory lactation access edits, service codes and consultant network data. Participate in DHCS guidance review and prepare to implement the no-authorization, no-prescription and no-referral instructions when applicable conditions and guidance permit.
