Acute Hospital Care at Home telehealth billing uses hospital POS 21
Oct 4, 2026 edition · GC-2026-0153 · Published 2026-10-04 · Version 2 · Final
CMS instructs AHCAH telehealth billing to use inpatient POS21 with modifier95 because the patient remains an inpatient.
What happened
The October 1 MLN Connects newsletter directs billing of Acute Hospital Care at Home telehealth services with place-of-service21, inpatient hospital, and modifier 95 for synchronous telemedicine. CMS explains that AHCAH patients remain hospital inpatients even when care is delivered through telehealth at home. The patient’s physical location therefore does not alone change the service to ordinary home telehealth coding.
This guidance addresses the AHCAH inpatient context. Teams should preserve that distinction when charges pass from clinical documentation through professional billing interfaces, rather than automatically deriving a home place of service from an address or remote encounter flag. As of October 4, the newsletter is a current coding reminder; it does not itself establish AHCAH participation, medical necessity or a different payer’s rules for every home-based service.

Why it matters
Automatic home-telehealth mapping can conflict with the inpatient status of an AHCAH encounter.
Recommended action
Validate AHCAH-specific POS21 and modifier95 mapping and professional billing interfaces, with documentation of the inpatient encounter and separate review of other payer requirements.
Implementation playbook
- Validate AHCAH charge mapping, professional billing interfaces and payer-specific claim edits with the hospital billing team. Revenue Cycle · Due 2026-10-09
- Record the applicability assessment and validate the affected workflow with its operational owner. This is an internal review target, not an additional statutory deadline. Compliance · Due 2026-10-12
