California IRF ownership changes must preserve RCD selections and identifiers

Noridian explains how an IRF ownership change affects RCD choices, affirmation history and portal/UTN identifiers.

What happened

Noridian’s September 28 guidance distinguishes accepting an existing Medicare provider agreement during an IRF ownership change from rejecting assignment and undergoing new-provider enrollment and survey steps. Where assignment is accepted in an active RCD state, the buyer continues the current review-choice selection and inherits the cycle’s reviews and affirmation decisions. Current and prior affirmation rates affect choices in the next cycle. Buyer and seller must coordinate if a selection period occurs before the ownership change is finalized, rather than assuming the transaction resets the demonstration history.

After finalization, Noridian directs use of the buyer’s current CCN/NPI combination for portal access and new unique tracking number requests. An NPI mismatch between the UTN and claim causes the claim to return, and a pre-change request needing an updated NPI should be raised with the Provider Contact Center. Administrators must also grant necessary staff portal permissions. Acceptance generally carries the existing provider agreement and CCN, so the instruction to use updated identifiers should not be read as proof every transaction creates a new CCN. As of October 4, RCD transition planning needs both inherited review status and consistent current billing identifiers.

Why it matters

An acquisition can preserve review obligations while changing access and billing identifiers, creating avoidable claim returns if those elements are not reconciled.

Implementation playbook

  1. Include RCD selections, prior affirmation rates, portal permissions and UTN/NPI continuity in IRF acquisition and enrollment checklists. Revenue Cycle · Due 2026-10-09
  2. Record the applicability assessment and validate the affected workflow with its operational owner. This is an internal review target, not an additional statutory deadline. Compliance · Due 2026-10-12
