CMS issues January ASP, MPFS and ZIP-code system update instructions
Oct 4, 2026 edition · GC-2026-0152 · Published 2026-10-04 · Version 2 · Final
CMS’s January 2027 reference-file instructions distinguish ASP pricing, MPFS dataset readiness and quarterly ZIP-file updates.
What happened
CMS issued three September 29 instructions with a first January 1,2027 effective date and January 4 implementation. CR 14620 supplies Part B ASP and NOC drug-pricing files, including possible prior-quarter revisions. The files contain the applicable payment allowances, so contractors must not add another payment calculation; unlisted drugs follow the referenced manual rules. The instruction says contractors need not search and adjust already processed claims unless brought to their attention. The January release therefore does not by itself guarantee automatic reprocessing of every affected older claim.
CR 14618 directs downloading and testing the annual physician-fee-schedule datasets, including anesthesia, when CMS notifies contractors around the final-rule display, and readiness for up to three replacement datasets before January 4 unless otherwise directed. CR 14623 supplies quarterly ZIP5/ZIP9 and year-end files for geographic pricing, with April, July and October effective dates distinct from their implementation dates. CMS anticipates files roughly six weeks before each quarter and recognizes that newly created postal codes may still require the manual exception process. As of October 4, these are file-handling and readiness instructions; actual payment policy, released file versions and later corrections must be checked separately.

Why it matters
Separate file owners and version controls are needed for drug allowances, professional payment and geographic pricing, including any prior-quarter corrections.
Recommended action
Inventory the three releases, plan vendor and boundary-date tests, and track CMS availability notices and replacements. Review specific older-claim issues rather than assuming an automatic adjustment sweep.
Implementation playbook
- Inventory the three releases and test the first January implementation. Monitor subsequent corrections and the applicable final payment policies separately. IT · Due 2027-01-01
- Record the applicability assessment and validate the affected workflow with its operational owner. This is an internal review target, not an additional statutory deadline. Compliance · Due 2026-10-12
