CMS replaces October EDI instructions to update the 276/277 flat-file attachment

CMS’s September 28 EDI replacement updates the 276/277 flat-file attachment while keeping October 1 effectiveness and October 5 implementation.

What happened

CMS rescinded September 18 Transmittal 13967 and replaced it with September 28 Transmittal 13977 for the October EDI front-end update, CR 14429. The latest cover states that the change updates the 276/277 flat-file attachment and that all other information remains the same. The September 18 predecessor had itself replaced the August 27 version to correct two typographical errors in the edits spreadsheet. These are successive revisions with different stated purposes. The current instruction is effective October 1, 2026, with systems implementation October 5.

The recurring update supplies common front-end HIPAA edits for Part A and Part B contractors and CEDI and directs shared-system updates. The requirements address TA1 and 999 acknowledgment handling, updated edit spreadsheets and the institutional and 276/277 flat-file layouts; the contractor change log identifies the quarterly modifications. Acceptance at the transaction-validation stage is not a claim payment or coverage decision. As of October 4, organizations should verify that vendor materials incorporate the final September 28 attachment and reconcile actual deployment with the October 5 implementation milestone instead of treating an older spreadsheet check as completion of the latest revision.

Why it matters

Version control across clearinghouse and billing interfaces can affect acknowledgment and claim-status handling even where underlying coverage is unchanged.

Implementation playbook

  1. Internal target: obtain the September 28 R13977 replacement and current 276/277 flat-file attachment; compare them with superseded vendor materials while preserving October 1 applicability. Revenue Cycle · Due 2026-10-04
  2. Internal target: regression-test current front-end and 276/277 claim-status scenarios with IT and vendors against the September 28 attachment. IT · Due 2026-10-05
  3. Validate actual deployment and acknowledgment/exception monitoring against the October 5 CMS implementation date; record unresolved exceptions and follow-up without assuming deployment is complete. Revenue Cycle · Due 2026-10-05
