FY 2027 IPF payment correction changes per diem and ECT amounts

CMS’s October 1 IPF correction lowers specified national payment amounts after wage-index recalculation.

What happened

CMS’s October 1 correction to the FY 2027 inpatient psychiatric facility rule is effective that day. The national per diem changes from $912.40 to $912.13, with the non-quality-reporting amount changing from $894.56 to $894.29. Electroconvulsive therapy payment changes from $688.59 to $688.38 per treatment, and its non-reporting amount changes from $675.13 to $674.93. The wage-index budget-neutrality factor changes from 0.9989 to 0.9986. These are corrected inputs to the adopted payment system, not a newly introduced IPF payment methodology.

The notice traces the error to an IPPS hospital wrongly treated as having converted to a rural emergency hospital, which excluded its wage data and affected the IPF wage index for the identified Mississippi area. Recalculation then changed national budget neutrality and payment amounts. CMS also corrects the poisoning comorbidity code count, the nonqualifying-emergency-department adjustment factor and impact analysis. As of October 4, an affected provider’s payment still depends on the appropriate geographic and facility factors and quality-reporting status, so checking only the national base amount is insufficient to reconcile claims.

Why it matters

Small national-rate corrections can create systematic discrepancies, while provider-specific factors determine the actual claim effect.

Implementation playbook

  1. Compare IPF rate tables and October claims against the corrected notice and verify quality-reporting adjustments. Revenue Cycle · Due 2026-10-09
  2. Record the applicability assessment and validate the affected workflow with its operational owner. This is an internal review target, not an additional statutory deadline. Compliance · Due 2026-10-12
