CMS issues FY 2027 IPPS and LTCH implementation instructions

CMS’s September 29 IPPS/LTCH correction and replacement instruction require corrected FY 2027 files and October 5 implementation checks.

What happened

CMS issued a September 29 correcting notice for the FY 2027 IPPS/LTCH rule, effective October 1, and replaced Transmittal 13930 with September 29 Transmittal 13969, CR 14592. The replacement revises its policy section and requirement 14592.13 to resolve typographical errors, with other information unchanged. It specifies October 1 effectiveness and October 5 implementation, generally applying the FY 2027 changes to discharges from October 1 through September 30, 2027 unless noted otherwise. The instruction directs contractors to use the correcting-document versions of tables and files when applicable, rather than assuming the original final-rule download remains current.

The correcting notice explains that an IPPS hospital had incorrectly been treated as converted to rural emergency status and another hospital’s canceled rural reclassification was misapplied. Restoring and correcting those data required recalculation of relative weights, wage indexes, budget-neutrality factors, standardized amounts, outlier and capital factors, as well as affected uncompensated-care calculations. LTCH area wage indexes also change because they use the same underlying wage data. The instruction includes Grouper and MCE version 44.0 and updated Pricer and provider-file work. As of October 4, teams should verify the corrected files and discharge-date logic and resolve implementation exceptions for October 5; this source review does not establish any organization’s deployment is complete.

Why it matters

A narrow source-data correction can propagate across payment factors, so finance and claims testing must reconcile the actual corrected file versions.

Implementation playbook

  1. Internal target: obtain the September 29 correcting-notice tables and R13969 replacement instruction; identify affected rates, grouper/MCE version 44.0, Pricer and provider-file inputs. Revenue Cycle · Due 2026-10-04
  2. Internal target: reconcile representative IPPS and LTCH test claims using corrected files and October 1 discharge applicability, documenting results and remaining exceptions. IT · Due 2026-10-05
  3. Validate actual corrected-file deployment against the October 5 CMS implementation date and reconcile financial exceptions; record status rather than assuming earlier production readiness proves this replacement is deployed. Finance · Due 2026-10-05
