2027 Medicare HCPCS quarterly instructions and clotting-factor fee distinguish service and implementation dates

CMS’s 2027 HCPCS instructions have quarterly service and implementation dates; the clotting furnishing fee is $0.270 per unit from January 1, with January 4 implementation.

What happened

CMS's August 27, 2026 transmittals distinguish the 2027 service dates from contractor implementation. R13938CP covers the January, April, July and October HCPCS releases: effective dates are the first of those months, while implementation dates are January 4, April 5, July 5 and October 4, 2027. Contractors retrieve files through the cloud after CMS email instructions, with updated files expected roughly six weeks before each quarter. This is an annual set of quarterly download instructions, not a rule that every 2027 HCPCS change becomes effective January 4.

R13937CP sets the 2027 clotting-factor furnishing fee at $0.270 per unit, effective January 1 and implementing January 4. CMS includes it in published national payment limits for clotting-factor codes. Where a separately payable factor has no limit in the ASP or NOC pricing files, the contractor pays for the factor and the furnishing fee; the manual also preserves the exception where furnishing costs are paid through another payment system. Payment logic must therefore distinguish an included fee from a separately added fee and use dates of service from January 1, rather than delaying applicability until the systems implementation date.

Why it matters

Service-date pricing and system implementation are different controls. A fee already included in a published limit should not be added again.
