MACs must post quarterly Targeted Probe and Educate denial trends
Oct 4, 2026 edition · GC-2026-0149 · Published 2026-10-04 · Version 2 · Final
CMS directs quarterly MAC TPE denial-reason postings from November 2 to support provider education and monitoring.
What happened
CMS’s September 30 Transmittal 13971, CR 14616, requires contractors to publish quarterly Targeted Probe and Educate results on their websites, effective and implementing November 2. The business requirement calls for the top or most common denial reasons for each prioritized problem, listing three where applicable; additional reasons are encouraged if useful. CMS explains that consistent public posting across contractors responds to industry interest in these results as a tool for understanding issues and trends. The new posting obligation belongs to MACs and is not a new quarterly report imposed on providers.
The revised manual places the posting instruction beside provider-error-rate review, which considers the nature and value of the problem and the provider’s history when determining corrective action. It also preserves inclusion of claims denied for failure to respond to documentation requests in the specified error-rate calculation. Published denial patterns can inform education and sampling, but they are not a finding that every organization has the same error or requires the same corrective action. As of October 4, compliance teams can assign responsibility for reviewing relevant postings once the November instruction takes effect and assess applicability against their own records.

Why it matters
MAC trends can focus internal review, but provider-specific facts and documentation determine whether a published issue affects actual claims.
Recommended action
Assign a quarterly review of relevant MAC postings, select documentation samples tied to applicable denial reasons, and distinguish observed organizational defects from general contractor trends.
Implementation playbook
- Assign a quarterly review of California-relevant MAC TPE postings and use applicable denial reasons to select documentation samples. Compliance · Due 2026-11-02
- Record the applicability assessment and validate the affected workflow with its operational owner. This is an internal review target, not an additional statutory deadline. Compliance · Due 2026-10-12
