Medi-Cal youth gender-affirming services: October 13 reimbursement and continuity requirements

DHCS’s September 30 letter sets October 13 reimbursement limits for specified youth gender-affirming services, with an existing-hormone transition and continuing ancillary-care duties. Managed care plans must address member notices, care coordination and policy updates; the guidance does not permit blanket cancellation of care.

What happened

DHCS issued All Plan Letter 26-018 on September 30 to implement the federal Medicaid and CHIP funding limitation scheduled for October 13. For service dates from then onward, it identifies specified gender-affirming surgeries, puberty blockers, hormone therapy and associated professional services that Medi-Cal managed care plans will no longer reimburse for affected Medi-Cal members under 18 and CHIP members under 19. Members already taking gender-affirming hormones as of October 13 have a transition through April 12, 2027; the plan’s responsibility is for the medical benefit. That exception does not extend to new hormone starts, puberty blockers or surgery.

Coverage continues for medically necessary treatment of other conditions, complications and services outside the listed categories, including office visits, laboratory tests, speech therapy and psychotherapy. Earlier service dates and Medi-Cal adults remain outside this change. Plans must help coordinate state-only HCAI funding, and the letter says providers should not cancel services or redirect members in the interim. It requires the approved handbook errata and, for affected prior authorizations, individualized adverse-action notices with the applicable ten-day timing, exceptions and appeal rights. Plans must update policies and submit them with the required confirmation within 90 days of the letter’s release.
