SNF FY 2027 correction revises wage-index calculations and burden estimates

CMS’s September 30 SNF correction revises wage-based payment calculations and separate QRP burden estimates, effective October 1.

What happened

CMS published a September 30 correction to the FY 2027 SNF payment rule, effective October 1. An IPPS hospital had incorrectly been classified as converted to a rural emergency hospital, excluding its wage data. Restoring the data required recalculating the SNF wage-index budget-neutrality factor and unadjusted and case-mix-adjusted federal per diem rates, along with worked payment examples. The notice changes the stated budget-neutrality factor from 0.9989 to 0.9988 and replaces the affected urban/rural rate and PDPM example tables. The displayed impact analysis is unchanged at its reported precision.

The same notice separately corrects QRP regulatory burden calculations that had used mean rather than median Bureau of Labor Statistics wages. Those labor-cost estimates describe the agency’s assessment of reporting burden and do not themselves add a new reporting requirement or change the adopted payment methodology. As of October 4, the relevant operational task is to reconcile the corrected wage and payment inputs while keeping burden estimates separate from claim rates. A single corrected factor cannot substitute for checking the complete applicable rate and facility inputs.

Why it matters

Correcting a wage-data classification can propagate through SNF payment tables, while burden-cost corrections have a different purpose.

Implementation playbook

  1. Reconcile the affected FY 2027 SNF rate tables and wage-index calculations against the corrected publication. Revenue Cycle · Due 2026-10-09
  2. Record the applicability assessment and validate the affected workflow with its operational owner. This is an internal review target, not an additional statutory deadline. Compliance · Due 2026-10-12
