AB 2160 directs updated Medi-Cal lactation guidance
Oct 11, 2026 edition · GC-2026-0251 · Published 2026-10-11 · Version 3 · Final
AB 2160 directs July 1, 2027 lactation guidance covering services, payment procedures, and plan-network information. The agency deadline and required consultation remain distinct from issuance of specific billing instructions.
What happened
DHCS must clarify the continuum from lactation education and basic support to clinical consultation, specify appropriate billing and reimbursement procedures, and clarify managed-care plans' coverage and payment responsibilities. The guidance must state that lactation services require no prior authorization, prescription, or referral. It must also require plans to report the number of accessible International Board Certified Lactation Consultants for members needing clinical consultation. These related items address both service access and the operational route for reimbursing the covered work.
The department must seek stakeholder input before issuing the updated guidance, and implementation depends on federal participation and necessary approvals. The act permits letters, bulletins, and similar instructions pending regulations. July 1 is the department's guidance deadline; it is not evidence that particular new codes, rates, or reporting templates already exist as of October 11. Providers and plans therefore need the actual instructions for implementation while preserving current coverage and clinical scope requirements.

Why it matters
Providers need the actual benefit and billing instructions rather than an assumed immediate payment change.
Recommended action
Review current lactation services and monitor the guidance; coordinate plan and provider implementation when the approved instructions are issued.
Published long lead justification
The July 1, 2027 guidance milestone exceeds 180 days, but stakeholder input, lactation-service inventories and billing/network readiness need advance coordination. Track agency and federal-approval dependencies without treating the guidance as already issued.
