Medi-Cal clinic institutional claims require diagnosis codes from October 15

Medi-Cal adds a clinically appropriate diagnosis requirement to specified clinic institutional claims submitted from October 15. The notice uses submission date, rather than establishing a new service-date cutoff.

What happened

The October 9 notice requires at least one clinically appropriate ICD-10-CM diagnosis on UB-04 or 837I claims submitted on or after October 15 by FQHCs, RHCs, IHS-MOA 638 providers, and Tribal FQHCs. It points to existing outpatient UB-04 completion instructions and says updated manual sections will follow in a future Medi-Cal Update. The scope is the listed providers and institutional claim formats; the announcement does not state that every professional claim changes or that older service dates exempt a claim submitted after the trigger.

Why it matters

The trigger is claim submission, rather than a new date-of-service limit. DHCS says corresponding provider manual updates will follow.
