HHS vaccine-category RFI’s September 20 assured-consideration date has passed
Sep 27, 2026 edition · GC-2026-0002 · Published 2026-09-27 · Version 2 · Announced
HHS’s vaccine-category RFI asks about routine, risk-based and shared-decision recommendations; its September 20 assured-consideration date has passed.
What happened
HHS’s August 24 request for information asks whether the categories used in federal vaccine recommendations remain adequate: routine or universal, risk-based and shared clinical decision-making, also called individual-based decision-making. It seeks views on evidence, category design, downstream legal and programmatic consequences and communication. The notice asks for comments by September 20 to assure consideration. That stated date has passed by this edition’s September 26 coverage end; the RFI itself does not establish a replacement category framework or change a particular vaccination recommendation.
The notice describes different defaults: routine and risk-based recommendations ordinarily favor vaccination absent relevant contraindications, while shared clinical decision-making has no default and depends on individual characteristics, evidence, clinical judgment and patient or parent preferences. Its questions about those distinctions and their consequences explain why order sets, patient explanations and coverage communications may need later review. The RFI remains an information-gathering notice rather than an independent mandate to alter clinical practice. Any operational change requires its own applicable authoritative recommendation or legal requirement.

Why it matters
Organizations should know where category terminology appears in workflows without treating an RFI or an elapsed feedback date as a new clinical policy.
Recommended action
Retain an inventory of category-dependent order sets and patient/coverage materials, monitor actual subsequent decisions, and verify any later opportunity before planning a submission. Continue applicable current recommendations and requirements.
