AHRQ publishes PSO delisting for cause

AHRQ’s October 2 notice confirms that Hospital Rep Safety Solutions Corp. lost its PSO listing for cause after failing to correct a deficiency. Delisting took effect September 9. Organizations using that PSO should review affected reporting arrangements and privilege questions with Legal.

What happened

AHRQ published a for-cause delisting notice for Hospital Rep Safety Solutions Corp., patient safety organization P0264. The notice states that the organization failed to correct a finding of deficiency and that delisting became effective at midnight Eastern Time, 2400, on September 9, 2026. Publication on October 2 is a separate date. This was a for-cause removal, rather than voluntary relinquishment or expiration of a listing; the notice does not describe the underlying deficiency in detail.

The Patient Safety Act and Patient Safety Rule establish a framework for providers to voluntarily report patient safety work product to listed PSOs for aggregation and analysis on a privileged and confidential basis. AHRQ administers PSO listing and operation, and the rule requires public notice when an organization is removed. That context makes the identity and listing status of a contracted PSO relevant to reporting arrangements. The notice does not decide the protection of every item previously submitted. Affected organizations should have Legal assess their specific arrangements and any reporting transition rather than assume that all existing work product automatically lost protection.
