CDPH export records sixteen historical September 21–25 facility citations
Oct 11, 2026 edition · GC-2026-0138 · Published 2026-10-11 · Version 4 · Announced
CDPH’s export contains 16 penalty records issued September 21–25 across 15 facilities. The records cover patient care, unreported abuse, material falsification and physical environment. They describe assessed citation metadata, not complete incident narratives, appeal outcomes or 16 new actions in October.
What happened
The retained CDPH State Enforcement Actions Details export identifies 16 penalty records with issue dates from September 21 through 25, covering 15 facility identifiers. Eight records are classified as patient care, six as abuse not self-reported by the facility, one as willful material falsification and one as physical environment. Intercommunity Care Center has two separate penalty records, which explains why the number of citations exceeds the number of facilities. The selection includes skilled-nursing facilities and other licensed facility types, rather than only nursing homes.
The same export shows an Open disposition for all 16 records, while some rows have a zero balance. Open therefore should not be read as proof of unpaid penalties, a pending lawsuit or a particular appeal result. The dates when penalties were issued also differ from the earlier violation dates recorded in the rows. These are historical September citations; the retrieved export contains no penalty-issue-date matches for October 4–10. The data supplies categories, identifiers, assessed amounts and related metadata, but individual citation documents remain necessary before describing what happened to a patient or drawing case-specific conclusions.

Why it matters
The additional records show patient-care, abuse-reporting and record-integrity exposure. Issued penalties are not proof of final appeal outcomes; the export does not supply full case narratives.
Recommended action
Facility compliance should review patient-care escalation, required abuse reporting and record accuracy. Obtain each citation document before drawing case-specific conclusions and track appeals separately from assessed amounts.
