OIG questions $454.9 million Wisconsin school-based Medicaid reimbursement
Oct 11, 2026 edition · GC-2026-0194 · Published 2026-10-11 · Version 4 · Final
OIG questioned an estimated $454.9 million in Wisconsin school-based Medicaid reimbursement, separating at least $67 million in estimated improper claims from $387.9 million the state may substantiate. Wisconsin generally disputed the findings. The combined estimate is not a court judgment or completed recovery.
What happened
OIG’s Wisconsin school-based services audit, A-02-24-01020, examined $505.5 million in reimbursement during a four-year period. Issued September 30 and posted October 2, it estimated at least $67 million in improper federal reimbursement associated with unsupported random moment time-study coding and services by unlicensed providers. OIG questioned an additional $387.9 million because the state could not support allocation ratios. Time studies estimate the portion of personnel activity attributable to particular services, making both coding and allocation support important to reimbursement.
The six recommendations include refunding the $67 million estimate and either refunding $387.9 million or providing documentation reasonably supporting the ratios. OIG also identified potentially unallowable costs because studies did not sample moments at the beginning of the school year. Wisconsin generally disagreed, agreed in part with the first finding, disagreed with the second and did not clearly state a position on the third. The conditional refund-or-substantiate recommendation must remain distinct from an unconditional established liability. Organizations comparing their own programs should examine time-study evidence, provider qualifications and allocation documentation together.
