What happened

California approved and filed AB 1906 on September 27. Affected commercial contracts and policies issued, amended or renewed on or after January 1, 2027 must cover cervical cancer screening ordered or provided by an in-network provider acting within scope and consistent with the referenced screening recommendations. The covered tests include Pap tests, FDA-approved HPV screening and FDA-authorized or cleared self-collected cervical screening kits usable outside a clinical setting. The text prohibits cost sharing for covered services under the section, subject to preservation of HSA-eligible high-deductible status. Specified specialized, vision-only, dental-only and Medicare supplement products are excluded; the commercial plan section excludes Medi-Cal managed care.

The separate Medi-Cal provisions address screening and authorized or cleared home kits on or after January 1, 2027, subject to medical necessity, utilization management and federal-law limits. Kits must be ordered or provided by the patient’s provider consistently with the screening recommendations and Medi-Cal policy, with clinically appropriate CPT or HCPCS billing. The kit provisions apply to fee-for-service and managed care, protect beneficiaries from cost sharing including share of cost or spend down, and depend on necessary federal approvals and available federal participation that is not jeopardized. DHCS may issue implementation instructions. As of October 4, neither the commercial contract date nor device authorization alone establishes readiness of every Medi-Cal payment pathway.
