What happened

California approved and filed SB 950 on September 27. Affected contracts and policies issued, amended or renewed on or after January 1, 2027 must cover medically necessary FDA-approved treatments or medications for Alzheimer’s disease or other conditions affecting memory, as determined by a health care provider. The protection reaches medical benefits and outpatient prescription benefits to the extent that outpatient drugs are covered. From January 1, step therapy is restricted for both self-administered and physician-administered treatments, but the statute does not require every treatment type to be available without it if at least one antiamyloid therapy is covered without step therapy.

Other utilization management, including prior authorization, remains permissible to determine medical necessity if applied in the same manner as for other covered illnesses. Coverage criteria may not be more restrictive than the FDA-approved indications, and qualifying authorization is treated as an exigent circumstance under the referenced review rules. The plan provisions exclude dental- or vision-only specialized plans, Medicare supplements and Medi-Cal managed care; the insurance section also lists limited-benefit policy exclusions. As of October 4, the January contract milestone calls for product and benefit preparation, while clinical eligibility and the specific step-therapy exception remain central to any individual request.
