2027 Star Ratings link enrollment planning to 2028 bonus payments

CMS's October 8 ratings inform 2027 enrollment and 2028 MA quality bonuses. Measure changes complicate year-to-year comparisons and do not establish an individual contract's performance without its actual results.

What happened

The 2027 MA and Part D ratings appear in Medicare Plan Finder for the October 15–December 7 enrollment period and affect 2028 MA quality bonuses. Contracts have different measure counts depending on MA-PD, MA-only, or standalone drug-plan status. The rating label therefore differs from both the underlying measurement periods and the later bonus-payment year.

New opioid/benzodiazepine and anticholinergic measures join returning or respecified functional-status and colorectal-screening measures. Physical- and mental-health improvement measures increase from weight one to three; selected older measures retire, and medication-review completion shifts to display before its planned return. These changes call for measure-specific review before attributing a changed overall score solely to improved or worsened care.

Why it matters

Improving physical and mental health measure weights rise from one to three; new measures and retired measures change comparisons. Do not confuse the rating year, underlying performance period and bonus year.
