California limits added billing barriers for Medi-Cal providers after other coverage pays

AB 1126 limits excess administrative barriers for residual Medi-Cal billing after other coverage pays. Its separate no-network-contract provision excludes Medicare coverage, while agreement exceptions, beneficiary protections, and federal implementation conditions remain.
What happened
For a noncontracted provider billing a Medi-Cal managed-care plan for allowable costs left unpaid by other coverage, DHCS must prevent requirements significantly exceeding those for billing the same costs through fee-for-service. A separate provision bars requiring a fee-for-service participating provider to become an in-network contractor to bill allowable covered costs when the other coverage is not Medicare. The Medicare exclusion belongs to that second provision; it should not be generalized to erase the separate comparative administrative-burden rule.
Plans may still require agreements for specified prior-authorization or other-payer-uncovered services and for qualifying continuity-of-care arrangements. Without an agreement, a provider may bear amounts above fee-for-service rates or applicable service limits, but cannot bill the beneficiary for excess unpaid amounts. DHCS may clarify billing, update guidance, and take specified administrative actions, with legislative updates from 2027 through 2030. Implementation depends on necessary federal approvals and available, unjeopardized federal participation. The statute therefore does not itself prove that all approval conditions have been completed or that every noncontracted claim is payable.

Why it matters
Plans need claims and provider-relations rules that accept eligible noncontracted-provider bills without adding prohibited network or administrative barriers.
Recommended action
Compare coordination-of-benefits edits, noncontracted-provider billing rules, and letter-of-agreement practices with the enacted limits, then prepare changes that activate after required federal approval and funding.
