Acute Hospital Care at Home telehealth billing uses hospital POS 21
Oct 11, 2026 edition · GC-2026-0153 · Published 2026-10-11 · Version 3 · Final
CMS ties Acute Hospital Care at Home telehealth billing to inpatient status, directing hospital POS 21 with modifier 95.
What happened
The October 1 newsletter instructs providers delivering telehealth to patients enrolled in Acute Hospital Care at Home to use place of service 21, inpatient hospital, and modifier 95 for synchronous telemedicine. CMS explains that these patients remain inpatients even when care is delivered at home. The instruction is specific to that enrolled hospital setting; it does not classify every home-based telehealth encounter as inpatient or establish a new ordinary home-telehealth billing rule.
