California IRF ownership changes require correct RCD selections and new identifiers

Noridian distinguishes IRF ownership changes that accept the existing provider agreement from those requiring new enrollment. Review-choice history, selection timing, and identifier matching remain consequential across the ownership transition.

What happened

The September 28 notice says the existing provider agreement is assigned to the new owner unless assignment is rejected. A rejecting buyer must complete the new-provider process and enters the target affirmation-rate review cycle applicable when enrollment is finalized. An accepting buyer continues the current RCD option and inherits reviews and affirmation decisions from the ongoing cycle; those results affect choices in the next selection period. Buyer and seller should coordinate any selection period occurring before the ownership change is finalized.

After finalization, the notice directs establishment of portal access using the buyer's applicable CCN/NPI combination and new tracking requests with updated identifiers. A UTN linked to an NPI different from the claim NPI causes return to provider. For a pre-finalization tracking request needing an NPI update, Noridian directs contact with the Provider Contact Center. Administrators must also authorize staff for pre-claim review and related functions. These steps distinguish continuity of a review-cycle choice from continuity of portal access and claim identifiers; inherited affirmation history alone does not resolve an identifier mismatch.

Assess the development with the operational owner before changing applicable policies or payment workflows.

Implementation playbook

  1. Include RCD selections, prior affirmation rates, portal permissions and UTN/NPI continuity in IRF acquisition and enrollment checklists. Revenue Cycle · Due 2026-10-09
  2. Record the applicability assessment and validate the affected workflow with its operational owner. This is an internal review target, not an additional statutory deadline. Compliance · Due 2026-10-12
