CMS explains the optional CCBHC state plan benefit
Oct 11, 2026 edition · GC-2026-0271 · Published 2026-10-11 · Version 3 · Announced
SMD 26-004 explains an optional CCBHC Medicaid state-plan benefit, distinct from the demonstration. Certification, service obligations, payment choices, and state approval remain necessary before treating the federal option as a local reimbursement pathway.
What happened
The October 6 letter implements authority enacted in 2024. Participating states certify clinics against current CCBHC criteria, with nine statutory service categories, additional required services, and ongoing encounter/outcome reporting. A community needs assessment informs staffing. Formal collaborating-provider arrangements may deliver services under the clinic umbrella, while access obligations include crisis availability and acceptance regardless of ability to pay or residence.
The state-plan option is distinct from the existing demonstration. A transitioning demonstration state may retain its payment methodology but must submit a state-plan amendment for CMS approval before implementation. The demonstration's enhanced matching rate does not automatically carry over to state-plan expenditures. The letter establishes a federal option and implementation conditions; it does not itself demonstrate California adoption, certify a particular clinic, or establish a provider-specific payment.

Why it matters
State certification uses current criteria, nine required services and ongoing reporting. CCBHCs serve people regardless of ability to pay or residence. The federal option is not evidence that California has adopted it.
Recommended action
Track California adoption and certification decisions; compare staffing, access and collaborating-provider agreements before making reimbursement assumptions. Playbook dates are recommended internal targets, except any explicitly identified agency date.
