CMS issues January ASP, MPFS and ZIP-code system update instructions
Oct 11, 2026 edition · GC-2026-0152 · Published 2026-10-11 · Version 3 · Final
Three CMS instructions coordinate January drug-pricing, physician-fee, and geographic reference-file releases. Their retrieval and testing requirements have different file scopes, while the ZIP instruction also establishes later quarterly release dates.
What happened
The September 29 ASP and MPFS transmittals use January 1, 2027 effectiveness and January 4 implementation. ASP instructions address current and revised prior-quarter drug files, separately distinguish OPPS processing, and direct contractors not to search previously processed claims unless brought to their attention. MPFS instructions anticipate datasets, including anesthesia conversion factors, around final-rule display in November and require readiness for up to three replacement datasets if corrections are needed. Those conditional replacements are not proof that three corrections have already been issued.
The ZIP instruction covers ZIP5 and ZIP9 files, including 2026 calendar-year-end files for the January quarter. Its effective dates are January 1, April 1, July 1, and October 1, with implementation January 4, April 5, July 5, and October 4. Contractors retrieve notified files into testing and production; ZIP5 supports ambulance pricing and other uses where directed. CMS warns even the latest file may omit ZIP codes created after compilation, requiring the manual's separate handling. These reference releases support payment processing without independently finalizing every underlying rate or coverage policy.

Why it matters
Different reference files feed drug payment, professional claims and geographic payment logic. A successful load requires coordinated vendor testing.
Recommended action
Inventory the three releases and test the first January implementation. Monitor subsequent corrections and the applicable final payment policies separately.
