DHCS prepares a January 1 transition to fee-for-service for specified immigration-status coverage
Oct 11, 2026 edition · GC-2026-0120 · Published 2026-10-11 · Version 3 · Final
DHCS separates the January 1, 2027 delivery-system transition for UIS members from loss of eligibility. Its September 29 provider notice preserves timely-renewal conditions and warns that managed-care-only providers need fee-for-service enrollment to continue serving affected members.
What happened
The September stakeholder announcement identifies January 1, 2027 for moving members with unsatisfactory immigration status from managed care to fee-for-service Medi-Cal. The September 29 notice introduces a provider resource hub with news, training, FAQs, and further materials as available, alongside member/provider guidance on DHCS's transition page. DHCS states that this changes how care is delivered rather than whether members remain eligible, while conditioning continued full-scope coverage on timely renewal. The announcement should not be generalized to every immigration-status group or treated as eliminating individual eligibility and renewal requirements.
The provider notice says organizations billing only managed care and lacking Medi-Cal fee-for-service enrollment cannot continue caring for affected UIS members after December 31, 2026. It directs providers to update PAVE enrollment, confirm current enrollment, screening, and revalidation, review fee-for-service billing and Treatment Authorization Requests, and update whether they accept new fee-for-service members. These specific conditions make enrollment and service-capacity checks material to continuity planning; a managed-care contract alone does not establish readiness for fee-for-service claims or authorization workflows.

Why it matters
Plans and providers need member, authorization, continuity-of-care, billing, and referral workflows that account for the coverage-delivery transition.
Recommended action
Confirm current fee-for-service enrollment through PAVE, pending enrollment and revalidation, service authorizations and continuity capacity before January 1. Coordinate member-specific transitions with plans and DHCS.
