FY 2027 IPF payment correction changes per diem and ECT amounts

CMS corrects FY 2027 IPF payment amounts after a wage-index error and separately clarifies treatment of awarded resident slots. Rate configuration and teaching-adjustment support require different checks under the retained replacement instructions.

What happened

The October 1 correction changes the standard federal per diem amount to $912.13 and the ECT payment per treatment to $688.38. For providers failing quality-data reporting, the corresponding amounts are $894.29 and $674.93. Restoring an incorrectly excluded hospital's wage data changed the wage-index budget-neutrality factor from 0.9989 to 0.9986, which flowed into these rates. The October 8 R14000 replacement retains the FY 2027 discharge period, October 1 effectiveness, and October 5 implementation while clarifying the FTE resident-cap language.

The manual allows the cap to increase from July 1, 2026 for resident slots awarded under section 4122 of the CAA 2023, including qualifying IPPS-hospital awards allocated to an IPF subunit. It does not permit affiliation agreements to aggregate IPF teaching caps. Less-than-full-time residents and partial-year rotations count proportionately, and resident time counted for IPPS indirect medical education cannot also support the IPF teaching adjustment. Payment uses the lower of the cap and actual resident count. An award thus needs allocation and time support rather than automatically increasing every teaching payment by the nominal number of slots.

Why it matters

Small rate differences propagate across claims, budget models and vendor configuration. Using the uncorrected final-rule table can produce systematic discrepancies. The cap may increase for awarded resident slots from July 1, 2026, including qualifying IPPS-hospital slots allocated to an IPF subunit. Do not double-count resident time under IPPS IME or aggregate IPF caps through affiliation agreements.

  2. Test cap and resident-time calculations against supporting cost-report records. Revenue Cycle · Due 2026-10-25
  3. Validate rate and teaching-adjustment configuration with the operational owner. Compliance · Due 2026-11-01
