IRF October reminder separates clinical timing rules, corrected rates and later QRP deadlines

The IRF rule sets admission-based clinical clocks separately from corrected FY 2027 rates and the later FY 2029 quality-reporting calendar.

What happened

The codified rule requires therapy to begin within 36 hours from midnight on the day of admission. CMS illustrates this with a Tuesday 2 p.m. admission and a Thursday-noon deadline, and explains that the clock concerns therapies ordered during preadmission screening; new therapies ordered after that initial period are outside this requirement. The initial interdisciplinary meeting is due on or before four days from admission, followed by meetings at least weekly using seven consecutive calendar days.

The correction sets the conversion factor at $19,866 and outlier threshold at $8,856 without changing methodology. Separately, January 2027 data preparation supports FY 2029 QRP submissions and corrections by the fifteenth day of the second month after quarter-end, with calendar exceptions and the influenza deadline preserved. These reporting dates do not extend clinical clocks.

Why it matters

Admission-based clinical clocks require precise timestamps. Corrected payment values and a later reporting calendar create separate configuration tasks; neither extends the time allowed for patient care.

Implementation playbook

  1. Audit all admission-ordered therapy starts and initial/subsequent team-meeting timestamps. Clinical Operations · Due 2026-10-18
  2. Verify corrected conversion and outlier values in payment tables. Revenue Cycle · Due 2026-10-25
  3. Configure the later QRP submission calendar and test assessment/NHSN reconciliation. Quality · Due 2026-11-01
