CMS issues FY 2027 IPPS and LTCH implementation instructions
Oct 11, 2026 edition · GC-2026-0123 · Published 2026-10-11 · Version 3 · Final
The FY 2027 inpatient/LTCH instructions require current corrected files, with October discharge rules and October 5 contractor implementation. A separate September 29 correction recalculates several payment factors after hospital-status errors, making hospital-specific reconciliation essential.
What happened
September 29 R13969 replaces R13930 to correct the Policy section and business requirement 14592.13 while leaving other information unchanged. The instruction retains October 1 effectiveness and October 5 implementation and generally addresses discharges October 1, 2026 through September 30, 2027 unless otherwise noted. It directs use of current final-rule or correcting-document tables and files, updated Pricer software, and version 44 grouper and Medicare Code Editor logic keyed to discharge date. Publication of those directions does not establish local installation.
The separate September 29 Federal Register document restores hospital CCN 250078, wrongly treated as converted to rural-emergency-hospital status, and corrects the rural classification of CCN 360068. CMS explains resulting recalculations of DRG weights, wage indexes, budget neutrality, outlier thresholds, standardized amounts, capital factors, and uncompensated-care factors, with related LTCH wage-index changes. These connected technical adjustments do not yield one uniform increase for all hospitals. Current hospital-specific files and actual October claim results remain the appropriate basis for reconciliation after implementation.
