Home health certification manual incorporates allowed practitioners

CMS incorporates allowed practitioners into home-health certification review while retaining eligibility and documentation controls. The November 2 manual change does not make agency-generated records alone sufficient evidence for payment.

What happened

The September 30 transmittal updates Program Integrity Manual chapter 6 to reflect CARES Act section 3708, with November 2 effectiveness and implementation. The revised manual recognizes physician assistants, nurse practitioners, and clinical nurse specialists as allowed practitioners for home-health certification and recertification. It retains homebound, skilled-care, plan-of-care, and face-to-face conditions. Failure to substantiate initial certification can also defeat later episodes even when recertification requirements are met, so accepting an allowed practitioner does not dispense with the underlying eligibility record.

The certifying practitioner's or qualifying acute/post-acute facility's record can establish eligibility, either alone or with appropriately incorporated agency documentation. Agency-generated records by themselves are insufficient. Supporting HHA material must be signed off and incorporated into the practitioner or facility record, corroborate the rest of the clinical record, and correspond to the billed dates. The manual treats incorporation as timely when signed off before or at claim submission. Its review approach therefore examines a consistent combined record, rather than accepting a stand-alone agency certification packet or rejecting valid records solely because an allowed practitioner supplied them.

Why it matters

Admission and certification controls should not reject valid allowed-practitioner records or relax the remaining homebound, skilled-care and face-to-face requirements.

Implementation playbook

  1. Validate certification templates, practitioner credentials and supporting medical records against the revised manual while preserving all other eligibility requirements. Clinical Operations · Due 2026-11-02
  2. Record the applicability assessment and validate the affected workflow with its operational owner. This is an internal review target, not an additional statutory deadline. Compliance · Due 2026-10-12
