Home health consolidated billing removes two supply codes in January

CMS removes A4453 and A4459 from the home-health non-routine-supply consolidated-billing list for service lines dated January 1, 2027 onward. The annual update adds no therapy codes. Removing these two codes from this edit does not establish coverage or eliminate other home-health billing requirements.

What happened

CMS’s October 8 transmittal 13988, CR 14639, removes A4453 and A4459 from the home-health non-routine-supply consolidated-billing code list for line-item service dates on or after January 1, 2027. The codes describe specified transanal-irrigation catheter and manual-pump enema-system supplies. Implementation is January 4. The annual instruction makes no addition to the therapy code list, so the two supply-code removals should not be generalized to an expansion of separately billable therapy.

Consolidated billing ordinarily directs payment for listed services during a home-health episode to the primary home-health agency, with the stated exceptions for physician-performed therapies and specified incidental or institutional supplies. CMS explains that recurring code-list changes enforce the existing benefit rather than redefine the services included in it. Billing teams should therefore change the two relevant edits by service date while preserving episode, coverage and documentation checks. A code’s removal from this list is not, by itself, a determination that every separately submitted claim for that product is payable.

Why it matters

No therapy-code additions are announced. The update changes coding used to enforce existing consolidated billing, rather than redefining the entire covered service set. Removal does not establish automatic payment or coverage.

Implementation playbook

  1. Map the affected claims and operative dates with the accountable owner. Coding · Due 2026-10-18
  2. Update the January exclusion table, test these two supply codes and retain other home health episode and coverage checks. IT · Due 2026-10-25
  3. Reconcile representative claims and retain implementation evidence before the operative release. Revenue Cycle · Due 2026-11-01
