MACs must post quarterly Targeted Probe and Educate denial trends

CMS directs quarterly MAC publication of TPE denial trends, with November 2 implementation. The publication duty gives providers information for targeted review while remaining separate from a finding about any individual organization's claims.

What happened

The September 30 transmittal seeks consistent TPE website postings across MACs. Its business requirement calls for quarterly results showing the top or most common denial reasons, listing three if applicable, for each prioritized problem. The revised manual describes at least the three most common reasons per problem; contractors should include additional reasons when useful to the supplier community. This is a contractor publication instruction, rather than a new quarterly report that every provider must submit.

The surrounding provider-error-rate guidance treats the nature, dollar value, and history of a problem as relevant to corrective action. It includes denials for failure to respond to additional-documentation requests in error-rate calculations and distinguishes prepayment allowable-charge and postpayment paid-error formulas. Those provisions explain why a published topic can guide documentation checks without proving a particular provider's error rate or sanction. An organization still needs its own applicable claims, records, and contractor communications before drawing that conclusion from a general trend posting.

Why it matters

Published denial patterns can guide prospective claims monitoring and education before an organization receives a TPE notice.

Implementation playbook

  1. Assign a quarterly review of California-relevant MAC TPE postings and use applicable denial reasons to select documentation samples. Compliance · Due 2026-11-02
  2. Record the applicability assessment and validate the affected workflow with its operational owner. This is an internal review target, not an additional statutory deadline. Compliance · Due 2026-10-12
