Medi-Cal RSV vaccine billing expands age eligibility and modifier rules

DHCS lowers code 90683's minimum age to fifty retroactively and applies SK only to ages fifty through seventy-four. The correction process concerns qualifying system-related denials, while age, modifier, and service-date checks remain separate.

What happened

For service dates from June 12, 2025, the notice makes the RSV vaccine code 90683 available from age fifty. Modifier SK is required for members aged fifty through seventy-four and is not required for members seventy-five and older. A single minimum-age edit therefore does not capture the full billing change: the age at service determines the modifier treatment as well as whether the updated minimum is met.

DHCS plans an Erroneous Payment Correction for appropriately submitted claims from the effective date that were denied because the system had not implemented the policy. Providers may instead correct and resubmit earlier denials using existing claim-inquiry and timeliness instructions. Updated manual sections are forthcoming. Those alternatives support reconciliation of actual eligible denials; they do not prove every denied vaccine claim will pay automatically or that the system correction has already completed.

Why it matters

DHCS will implement an erroneous-payment correction for appropriately submitted claims denied because system changes were missing. Providers may also correct and resubmit under existing timeliness instructions.

Implementation playbook

  1. Set age-specific SK edits and test examples at ages 50, 74 and 75. Coding · Due 2026-10-18
  2. Inventory denials from June 12, 2025 affected by the system lag. Revenue Cycle · Due 2026-10-25
  3. Reconcile EPC payments with corrected submissions to prevent duplicate recovery. Revenue Cycle · Due 2026-11-08
