Medi-Cal youth gender-affirming services: October 13 reimbursement and continuity requirements

APL 26-018 pairs youth reimbursement limits with a restricted hormone transition, preserved services, continuity coordination, and plan notices.

What happened

The September 30 letter limits reimbursement from October 13 for specified gender-affirming surgery, puberty blockers, and hormone therapy for Medi-Cal members under eighteen and CHIP members under nineteen. Hormones already being taken as of October 13 retain a transition through April 12, 2027; for affected MCP members, this applies only under the medical benefit. Surgery, blockers, and newly started hormones receive no such transition. Services for other medical indications and treatment of complications remain covered when medically necessary.

The letter preserves necessary ancillary services, earlier service dates, and unaffected ages. It directs coordination with HCAI state-only funding and says providers should not cancel or redirect care while reimbursement details develop. Plans must distribute approved handbook errata and issue individualized adverse-action notices when required, with ten-day advance notice where applicable, appeal rights, and continuation information. Policies, attestation, and copies are due within ninety calendar days of release, rather than requiring subsequent DHCS approval.
