Medicare SNF place of service utility will reprocess overlapping claims
Oct 11, 2026 edition · GC-2026-0273 · Published 2026-10-11 · Version 3 · Final
CMS will reprocess specified professional claims billed with nursing-facility place of service despite overlapping covered stays. The utility targets May 2023 through December 2024 service dates and can produce recoupments. April 2027 implementation gives affected organizations time to reconcile coverage and coding evidence.
What happened
CMS transmittal 13995, dated October 8, 2026, directs a one-time claims utility under CR 14582. Effective April 1, 2027, with April 5 implementation, it identifies professional claim lines using place-of-service code 32 for May 11, 2023 through December 31, 2024 when their dates overlap a posted Part A SNF or swing-bed claim. Critical access hospital swing-bed claims are excluded. Contractors must reprocess claims flagged by informational response 7303 and follow standard recoupment procedures for resulting overpayments.
The underlying distinction matters because POS 31 applies during Part A-covered SNF stays, while POS 32 covers nursing facilities and SNF stays without Part A coverage. In mixed facilities, CMS requires POS 31 unless the physician verifies no Part A payment will be made. Organizations should match potentially affected professional claims to actual covered-stay records before reconciling adjustments. The instruction assigns the historical search to contractors; it does not tell every provider to rebill all nursing-facility services.

Why it matters
POS 31 reflects Part A covered SNF stays, while POS 32 reflects nursing facilities or noncovered SNF stays. Reprocessing can create recoupments. The actual PDF identifies CR 14582, while its landing-page reference differs.
Recommended action
Preserve the PDF identifier discrepancy and inventory potentially affected professional claims before MAC adjustments. Playbook dates are recommended internal targets, except any explicitly identified agency date.
