Noridian coverage updates include November tendon-injection conformity revisions

Noridian pairs November 1 tendon-injection coverage and coding revisions. Its October 8 notices describe administrative conformity to the established chronic-pain nerve-procedure policy, rather than blanket injection coverage.

What happened

The published November 1 revisions pair L34218 revision 7 with A57079 revision 6 and describe administrative conformity to the chronic-pain peripheral-nerve policy L40265. The tendon/ligament/ganglion LCD addresses injections used within an overall treatment plan. Symptoms should resolve or be reevaluated after one to three injections; more than three injections at the same local site within six months require clinical justification for prior failure and an expectation of further benefit. The policy also questions repeated injections or more than two sites per session and excludes the described dry-needling procedures.

The companion billing guidance requires records supporting the diagnosis and decision to inject, additional justification for repeat treatment, and compliance with applicable correct-coding edits. Insufficient supporting documentation can lead to a medical-necessity denial. November 1 remains the future revision date as of October 11; these published documents should not be represented as already replacing the October version. Clinical justification and coding review remain separate from simply loading a revised article number. The public extract does not establish that every license-gated code table or individual claim satisfies the policy.

Why it matters

These are administrative conformity revisions under the listed JE Part B contractors, including California. The notices do not create blanket coverage for injections. The four earlier nerve-procedure and molecular-diagnostic updates remain a separate carryover research checkpoint pending current verification.

Implementation playbook

  1. Compare L34218 and A57079 conformity changes with operative L40265 criteria. Coding · Due 2026-10-18
  2. Recheck prior notices and test affected service-line documentation and edits. Utilization Management · Due 2026-10-25
  3. Verify claim configuration before the November effective date. Revenue Cycle · Due 2026-10-30
