October Medicare and Medi-Cal claims updates: reconcile completed implementation

October payment and coding updates require several distinct date and adjustment rules. Current EDI, SNF therapy-file placement, home-health grouping, and laboratory edits must be reconciled with the specific automatic Q5161 adjustment and Medi-Cal receipt-versus-service triggers.

What happened

The current EDI instruction is September 28 R13977, replacing R13967 to update the 276/277 attachment while retaining October 1 effectiveness and October 5 implementation. Other October releases use distinct triggers. MPFS R13929 amends 2026 payment files, with January 1 general effectiveness and individual-record dates. CLFS/CLIA uses CR 14569 and MM14569, effective October 1 and implemented October 5. SNF MM14561 removes specified therapy codes from Part B File 1 while retaining them in File 4, with October 5 placement dates in Table 8. Home-health R13793 requires grouper 07.2.26 for claim From dates on or after October 1 for bill type 032x except 32A and 320. These contractor milestones require release verification rather than an assumption that every local system installed the files.

Adjustment instructions also differ. SNF, MPFS, and CLFS sources describe adjusting affected claims brought to the MAC's attention without a general file search, whereas OPPS R13978 specifically requires Q5161 claims received July 1 through July 20 to be adjusted within 30 days of implementation. OPPS coding/payment changes do not themselves establish Medicare coverage. DMEPOS MM14619 separates October code and fee changes from January 2028 CGM/pump bundling and its patient-owned-equipment transition. Medi-Cal NCCI edits use claims received from October 1; Q4 HCPCS changes generally use service dates from October 1 unless specified otherwise. Reconciliation must therefore preserve each source's claim population, retroactive dates, and exception rather than applying one October deadline or reopening rule to every claim.

Why it matters

A contractor implementation date is not a provider filing deadline or proof that a vendor applied the release. Medi-Cal NCCI edits turn on claims received from October 1; Q4 HCPCS changes generally turn on dates of service from October 1. Coding and payment changes do not establish coverage, and retroactive corrections retain their separate dates.
